Incident Reports
Use Incidents to keep a consistent record of accidents, injuries, illnesses, behavioral events, and other reportable occurrences.
File a report
Select File Report, then record the child, incident type and severity, when and where it happened, what happened, action taken, and any witnesses. Saving creates a Draft report.
Review names, times, and the narrative before signing. A signed report’s original narrative cannot be edited. If information changes later, add an addendum so the correction remains traceable.
Record family notification
Use the notification action to record when the family was told, how they were contacted, and which guardian was reached. The Incidents page can be filtered to show reports where family notification is still pending.
Sign, add an addendum, or void
- Sign after the report is complete and reviewed.
- Add an addendum to a signed report to document a correction or later development.
- Void a report only when it should not exist, and enter the reason.
Access depends on the user’s incident permissions. Grant update access only to people authorized to sign reports and record family notifications.
Export records
Use the Incident Log in Reports for a date-range export. Follow your organization’s retention and notification policies; Kidago does not replace required regulatory reporting.
